Patient Registration

glasses_and_penPatients who wish to register with our practice will need to:

  1. Bring proof of id (passport,international ID card issued by EU country, driving licence)
  2. Bring recent proof of your address (e.g. a utility bill from the last 1 month), and
  3. Complete the relevant forms (these can be collected from the reception or downloaded from our website- please see below)

Hucknall Road Medical Centre encompasses a close catchment area, however under new arrangements introduced from January 2015 we now have the option to register patients who live outside the practice area. Please note that out of area registration is voluntary and you may still be refused because you live outside of the area we cover.

Local Practices

For further information regarding practices that cover your address or help in finding a practice to register with, you can contact NHS Shared Business Services on 01162 957880 or visit NHS Choices website at www.nhs.uk 

If a practice, including ourselves, removes you from their list due to a change of address, NHS Shared Business Services will not normally contact you.  Therefore it is your responsibility to ensure you and those residing at the same address are registered with a doctor's practice.
